Community Data Roundtable brings communities together through data. To accomplish this, we establish our Community Data Reports, examples of which you can see below. These reports are tailored to the specific needs of the communities with which we work, and the dynamic reports below are just a sample of what can be done. These visualizations are then reviewed in our regular “Community Roundtables,” this results in further drill-downs, identifications of opportunities and needs, and collaborative system planning.
Demo Provider Dashboard
The Demo Provider Dashboard demonstrates a dashboard used by providers in a two county system where CDR is implementing its model. This information allows each provider to be compared in regards to how many clients they are treating, as well as the Autism Severity breakdown of the population treated. The second visualization in this dashboard looks at the same flow of clients, but splits up the data in regards to Risk Severity, instead of Autism Severity. Filtering for age and date of measure is also included.
Demo Community Report
The Demo Community Report is for a five county collaborative which is implementing CDR’s model. There are many visualizations here, designed to help the Roundtable Stakeholders to understand their networks, and compare providers. There is much information here, including information about the average prescription of an important community based service (“TSS”) (please just put your mouse over one of the columns on the first graph to see this information “pop up”), Information on the most popular services prescribed in these areas (see Graph 4 – “Most Common Prescriptions”), and sophisticated analysis of clinician adherence with decision support algorithms (Graph 5 – CSM Analyses).
